Each of the programs is different and has different eligibility criteria, but the following information will most likely be required by each and may be helpful for you to gather as you begin your inquiry:
- Your doctor's name and contact information (most programs require your doctor or health care provider's assistance).
- The name and manufacturer of the medication(s) you are taking.
- Your financial information including, proof of income, tax returns, social security income etc. Some programs require meeting specific low-income guidelines, others look to total health care costs compared to income/family size/living expenses.

Provides co-pay assistance to those who already have some form of medical insurance. Eligibility is NOT income dependent, but based on a comprehensive financial assessment of how much income is going toward medical bills. Applications are taken on the first business day of the month by calling 1-866-512-3861. Check the website to see if your particular cancer is covered, and know that new diagnoses are added on an ongoing basis. Chemotherapy and other medications are covered for certain types of cancer.

PSI offers co-pay assistance for specific types of cancer. They can be contacted at 1-800-366-7741 or by clicking on the link above. Eligibility is based on an overall assessment of a patient's financial income and medical expenses. PSA can also help eligible people with costs for COBRA or other insurance coverage.

The Leukemia & Lymphoma Society’s Co-Pay Assistance Program offers assistance to patients with lymphoma, myeloma, AML, CLL, Waldenstrom Macroglobulinemia, and MDS who are at or within 500% of the federal poverty level. It offers assistance to patients toward private health insurance premiums, private insurance co-pay obligations as well as Medicare Part B or D, Medicare Supplementary Health Insurance and Medicare Advantage.
